Cooperative agreement to build and sustain evidence-based chronic disease self-management for older adults with behavioral health conditions
This cooperative agreement will fund up to five organizations to design and implement strategies that improve how evidence-based chronic disease self-management education (CDSME) programs are delivered and sustained. The focus is on helping older adults who also have one or more behavioral health conditions. Projects are expected to be multi-faceted and support long-term program viability.
